OCOM 6009 - Longitudinal Integrated Clerkship 3 The third in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, as well as, patient-centered medical home practice.
Course Outcomes - Explain the benefits of interdisciplinary health care teams.
- Identify the communicative needs of consultants and other parts of the health care system.
- Assist patients in connecting with peer support groups or other appropriate community resources.
- Assist patients with health behavior change.
- Apply team-based principles to patient care.
- Support and visit a panel of patients in a longitudinal fashion.
